Lucky 13 Tattoo Inc. requires all customer to have a photo ID.
Lucky 13 Tattoo Inc. reserves the right to refuse services to those customers not providing proper Identification.

Lucky 13 Tattoo Inc. does provide services to minors with parents present and proper identification. Both minors and parents must have a photo ID. The state of Kentucky requires that any release forms for minors be notorized. Lucky 13 Tattoo Inc does have a notary present on premises to notorize these forms. Tattooing is offered to minors age 16 and up and limited body piercing is offered to for those 15 and up. Remember, you MUST have proper identification!

Proper Identification for minors includes a state issues id or a photo school ID and birth certificate
